Log notes

  • Follow your clinician’s schedule — daily or a few days a week
  • Sit, feet flat, arm supported; avoid caffeine just before if they say so
  • Bring the list to Bay check — do not self-adjust meds from this page
  • Chest pain, severe headache, or sudden symptoms — urgent care, not this crib
